I tried to mount an Altec 291B driver to a Mantaray 594II horn and to much surpise, the holes don't match up. The Horn has "blanks" or fake holes where the bolts are on the 291's. Are these there to be drilled out? or do i need some sort of adapter?

You need an adapter plate. It's a flat metal plate drilled for both the bolt patterns.
The indentations are for the nuts holding the driver to the plate, which then bolts to the horn.
The horn in this auction has one.

If you can't find a pair the bolt hole dimensions are in this thread, a machine shop should be able to make them. They show up on eBay often.

If you look carefully at your 594 horn, you will see that it is made in 3 pieces which are then glued together. The adapter plate is absolutely essential to carry the physical weight and stresses created by the mass of a 288/291/299 driver. Keep this in mind when mounting.

Here's my MR II-594 story in pics:
No studs in the 291's, short bolts are used instead. Studs are too long for the pockets in the horn flange i think.
